ONLINE FORMS AND DELIVERY INFORMATION

We need verification of contact lens wear before we can fulfill your order:

1) All new clients please fill out the Patient Intake Form and upload your latest Contact Lens Prescription. https://form.jotform.com/232698853517167

2) Anyone under 18 years of age will need the Parental Consent Form filled out as well as the Patient Intake Form.  https://form.jotform.com/233356439326056

3) If you are a preexisting client and we have you records on file please message us ON FILE with name and Date of birth in the chat box or email us at cosmiccontactlenses@gmail.com.

New to contacts and DO NOT have a prescription?

We have 3 buying options for you:

1) Visit your local Board-Certified Licensed Optician or your Eye Doctor to receive a contact lens prescription. We will hold your contact lenses until verification is complete.

2) Presale pick up at future events or conventions. If contact lens fittings are needed with measurements they can be done on site for a fee.

3) South Florida Mobile Delivery. We provide contact lens measurements and basic insertion and removal training within Palm Beach County for a minimum fee of $55 plus the contact lens purchase. This fee includes the Insertion and Removal process of the contacts, and tips from a Board-Certified Licensed Optician.
